    Hello everyone.  I am looking for teacher/staff evaluation forms.  What do all of you use?  Do you have your teachers/staff do a self-assessment as well?

    Hi Tera,
    I always like doing self-evaluations as they can be a valuable tool to understand ourselves and the expectations of us.  Attached is a self-evaluation developed by Exchange Press (a great resource!) that we liked for quick usage.  Also attached is a teacher assessment we developed for ourselves for formal evaluations.  We have teachers self-assess using the one we developed for ourselves prior to giving appraisals.  We also have a separate evaluation of how they are as a general employee (not related to their position).

    Hello!
    I like to use a running record/anecdotal format for teacher evaluations.  It isn't a checkbox system and won't give you list of skills a particular teacher can successfully use in the classroom.  Instead, it's a learning story about the teacher.  Evaluations are a means to understand what and why you are doing in the classroom and how to look at it through an observer's eyes.  First, I observe a teacher in a classroom for 20 minutes using a running record and taking pictures to demonstrate what is happening in class.  After the observation, I review what was observed then ask the teacher questions about what I saw and how they would explain their actions.  The third part is sending the documentation to the teacher for her feedback--adding her background and answering the questions posed.  Once the document is complete with all three parts--the running record of the observation, the questions asked to understand the why of what happened, and the feedback from the teacher upon receiving this learning story--I sit down with the teacher to have a discussion about their evaluation and to clarify anything that may seem out of context or character.
    Learning stories are great ways to document what a teacher is doing in the classroom and allows the teacher a chance to respond to what you see through an observer's eyes.

    Whatever evaluation you choose, remember that is only a snapshot of what the teacher did on that given day and does not accurately asses everything of which the teacher is capable.

    Thanks for sharing your observation/evaluation protocol, Denise. This is really thorough, thoughtful and collaborative. I've been wondering how to engage staff more in my observational narratives, and this really helps. One thing I have started to do is to put a Question to consider at the end of my observation record to get our conversations started. I really appreciate how much work you put into this process!

    Thank you so much for the advice.  I am new to this position and used to having evaluations performed on me yearly.  This will be a first for my group of teachers.  If you could send me what you have that would be very much appreciated.
