Our athletics department uses a QR code (you can create them for free online) that the students scan using their phones. It links to a google doc where they answer the screening questions. All staff does is take the temperature of the student and the student inputs the rest, which is also time-stamped when they enter the data. I haven't tried this yet at our preschool, but it could be used in the same way where we take their temp and parents input the data. It is a very quick process and is also nice in that you have an electronic copy for record-keeping; however, unless the questions are also answered verbally by the parent you wouldn't know their responses to the additional questions unless you were monitoring the document in real-time. Pros and cons to doing it this way but it may be worth exploring.

I am curious to know if anyone (center/school) is using a health screening questionnaire app for child check in? We are trying to cut down to the amount of time it takes since we are doing a kiss and ride drop off. I would love to know how other centers are doing it, what works, and what doesn't.
